State of Minnesota group term life insurance plan

The life insurance plans made available to you and your family are an important building block in the foundation of financial security. They provide a base of protection that can be supplemented by personal savings, individual life insurance and Social Security benefits.

Basic life
  • Automatically enrolled for this coverage upon becoming benefits-eligible, unless employment status does not qualify you for employer provided coverage.
  • All coverage is guaranteed, evidence of insurability (EOI) is not required.
  • Manager's Plan
    Choice of 1.5 or 2 times your annual salary.
  • Other employees
    Refer to your collective bargaining unit or plan, to determine your amount.

Optional term Life

  • Elect coverage in $5,000 increments not to exceed $500,000.
  • As a new hire, you can elect up to two times your base salary, guaranteed - evidence of insurability (EOI) is not required, if elected within 35 days of your hire date.
  • If you become benefits eligible after your hire date, you can elect up to two times your base salary, guaranteed - EOI is not required, if elected within 30 days of your initial eligibility date.
  • EOI is required for coverage elected after your initial eligibility and/or if it exceeds the guaranteed maximum (two times your base salary).
  • Employees can elect this coverage at any time during the year.

Spouse term life

  • Elect coverage in $5,000 increments not to exceed $500,000.
  • As a new hire, you may elect up to $10,000 of coverage - guaranteed, evidence of insurability (EOI) is not required, if elected within 35 days of your initial eligibility date.
  • If you become benefits eligible after your hire date, you can elect up to $10,000 of coverage - guaranteed, EOI is not required, if elected within 30 days of your initial eligibility date.
  • EOI is required for coverage elected after your initial eligibility, and/or if it exceeds the guaranteed maximum ($10,000).
  • Employees can elect this coverage at any time during the year.

Child term life

  • Elect $10,000.
  • One premium insures all of your eligible children.
  • Children are eligible from live birth to age 26.
  • As a new hire, elect coverage guaranteed within 35 days of your initial eligibility date.
  • If you become benefits eligible after your hire date, elect coverage guaranteed within 30 days of your initial eligibility date.
  • After your initial eligibility period, coverage may be elected each annual enrollment period  guaranteed - EOI is not required, or coverage may be elected any other time during the year with EOI.
  • If two employees are married, and both are employees of the State of MN, only one of them can cover children.

Optional Accidental Death & Dismemberment (AD&D)

  • Employee coverage may be elected in $5,000 increments, up to a maximum of $200,000.
  • Spouse coverage:
    • May be elected up to a maximum of $25,000
    • Cannot exceed the employee’s amount
    • Terminates when your spouse reaches age 70
  • All coverage is guaranteed, EOI is not required. Employees can elect this coverage at any time during the year.

Term life coverage is offered under policy form series 03-30522 Rev. 1-2006, Optional AD&D coverage is offered under policy form series MHC-96-13240 Rev. 1-2006.
