As required by Florida Statutes §§ 627.4215 and 641.31085, all health insurers and managed care organizations must provide the state and federal requirements for coverage of behavioral health care services to all Florida residents under an insurance policy issued in Florida. Please note that, although we are providing this notice to you, it does not necessarily mean you have coverage for mental health services through your policy with us.

The state and federal requirements for coverage of behavioral health care services can be found on the Florida Department of Financial Services Mental Health website. Individuals needing assistance with mental health or substance use disorder benefits can also contact the Florida Insurance Consumer Helpline at (877) 693-5236 (statewide) or (580) 413-3089 (out-of-state).
