Basic life $10,000
  • Evidence of insurability (EOI) is not required
  • Age reductions apply*
Group term life One, two, three, four, five, six or seven times your salary ($1,000,000 maximum)
  • As a new hire, up to $700,000 is guaranteed - EOI not required - if elected within your waiting period
Spouse/domestic partner term life $10,000, $25,000, $50,000, $75,000 or $100,000
  • All spouse/domestic partner coverage is guaranteed - EOI not required - if elected within 45 days of initial eligibility
Child term life $10,000 or $20,000
  • All child coverage is guaranteed - EOI not required - if elected within 45 days of initial eligibility
  • Children are eligible from 14 days to 19 years old
  • One low premium insures all eligible dependent children
Voluntary accidental death & dismemberment (AD&D)

Employee only

  • $25,000 increments up to $350,000

Family plan

  • Spouse/DP (with children): 50%
  • Spouse/DP (no children): 60%
  • Each child (with spouse/DP): 15%
  • Each child (no spouse): 20%
  • The family plan provides a percentage of coverage for each dependent based on the employee’s elected amount
  • Age reductions apply*
  • You must have group term life coverage to elect voluntary AD&D

*Coverage reduces to 65 percent at age 65, 45 percent at age 70, 30 percent at age 75 and 20 percent at age 80
